Any Municipal or Ward Chair can submit an appointment request to the County Chair. These appointments, once received by headquarters, will be checked for eligibility and confirmation will then be sent back to the appointee and the requesting chair.
Any requested change to the committee list must use the appropriate form and include the appropriate documentation, if necessary. The form can be downloaded by clicking the link below.
Completed appointment forms can be emailed to firstname.lastname@example.org or mailed to HQ. Forms submitted using other means will not be accepted. Please feel free to contact HQ with any questions.
